Getting started
A clear place to begin, wherever you are in your care journey.
Do I need a referral?
No referral is required. Contact our team to discuss your situation and arrange an initial consultation.
How do I become a patient?
Call 604-428-1991 to arrange an initial consultation with Dr. Lemmo. Our new-patient information explains the intake form and health records to prepare before your appointment.
How soon can I have my first consultation?
We understand that time matters in cancer care and work to accommodate patients as promptly as possible. Call our office to discuss your circumstances and current appointment availability.
Can I have a telephone or virtual consultation?
Telephone and virtual consultations are available for people who cannot travel because of their health or distance. Contact our team to discuss the options for your situation and location. Please complete your intake form and provide relevant health records before the appointment.
Can I seek care if I do not have cancer?
We sometimes see people with other chronic health conditions and also support caregivers and family members of patients living with cancer. Call us to discuss whether the clinic is a suitable fit for your needs, or explore our health support services.
Your appointment
What to expect and how to prepare for your first conversation.
What happens at my first consultation?
Please allow at least one hour for your initial consultation and evaluation. You will discuss your diagnosis, health history, current care and goals with Dr. Lemmo. This helps shape an individualized plan, which may include further assessment or testing when indicated. Your questions are an important part of the conversation.
What should I prepare before my appointment?
Complete your intake form and gather relevant lab results, scans and oncology reports. Write down your main questions and your current treatments, medications and supplements. Our new-patient page explains how to prepare, and the clinic can confirm which records are needed.
What if I am travelling from outside Vancouver?
We welcome patients from across Canada and internationally. Contact us before making travel arrangements so we can discuss appointment planning and any special arrangements. Possible weekend services must be confirmed directly with the clinic. Our travelling to the clinic page includes information about the area.
What if I need to cancel or change my appointment?
Please call 604-428-1991 if you need to change your appointment. Our financial policy requires at least 24 hours’ notice for cancellations and describes the charge for a missed visit. You can review the policy in the patient intake information.
Care & support
Your questions, your healthcare team and support for the whole person.
How is my care plan developed?
Your diagnosis, health history, current treatments and personal goals guide the conversation. Dr. Lemmo will discuss the approaches that may be relevant to your situation and any assessment needed. Read more about our approach to integrative cancer care.
Will Dr. Lemmo work with my existing healthcare team?
Yes. Dr. Lemmo works with other healthcare professionals, including oncologists, surgeons, medical doctors, pharmacists, naturopathic physicians and other practitioners. Tell us who is involved in your care so collaboration can be discussed as part of your plan.
What if my oncologist or pharmacist has questions about a proposed treatment?
Please raise those questions with Dr. Lemmo and your treating team so the proposed plan, evidence and possible interactions can be discussed together. Ask the healthcare professional to call our office directly at 604-428-1991. Telephone discussion is preferred to email or letters for clinical concerns. You can also arrange an in-person visit to review your questions and the next steps.
Can the clinic provide a report for another healthcare professional?
Yes. Reports can be prepared at a patient’s request for other healthcare professionals. Contact our office to explain what is needed and who should receive it.
Where can I find emotional support?
Our emotional support page introduces Samira’s individual mentorship and provides her contact details. Visit the page to learn about the support offered and contact her directly to discuss whether it suits your needs.
Where can I learn more before my consultation?
Explore Dr. Lemmo’s video library and the studies linked on our treatment pages. These resources can help you identify topics and questions to discuss with your care team. You do not need to have every answer before contacting us.
Forms & records
Help with patient intake, medical records and ongoing refill requests.
How do I complete the patient intake form?
Open the patient intake page or use the guided form on the new-patient page. Your identity and contact details, practice acknowledgment, full name, electronic signatures and signing dates are required. Health and background questions are optional. Share the information you can, review the practice information and add any supporting records before submitting.
Can I use a PDF or keep a copy of my intake?
Yes. An optional fillable PDF is available on the new-patient page. After submitting online, look for the completed PDF download and save a copy for your records. Contact the clinic if you need help completing or submitting your form.
How do I send medical records or request their release?
You can attach supporting records to the online intake form. If the clinic needs to request records on your behalf, contact our team for instructions and ask which documents are needed. Please do not send medical records through the general enquiry form.
How do I request a refill?
Use the patient refill request form to list the supplements, pills or other clinic products you need, including their strength if known and the quantity requested. Our team will call to confirm the items, shipping and payment before finalizing your order.
