LEMMO Integrated Cancer Care Inc. welcomes patients locally, throughout Canada, and internationally. No referral is required and due to the time sensitive situations in cancer care, we work hard to accommodate patients as fast as possible.

You will be required to fill out and submit our New Patient Intake Form (begin online from below) and submit your pertinent health records (i.e. lab tests, scans, oncology reports). Once you complete the form and submit, you will receive a notification on the webpage that your submission was successful. Medical records can be uploaded at the end of the form and submitted online, or sent to our clinic by email or fax (604.428.1992).
