Contacting LEMMO Cancer Care
Our general enquiry forms ask for your name, email address, optional telephone number and message so our clinic can respond to your enquiry. You may also attach one general document in PDF, JPG, PNG or ZIP format, up to 18 MB. Your enquiry and any attachment are emailed to info@lemmo.com for our team to respond. Please do not include medical records, test results or other sensitive health information.
The patient refill request form asks for your name, email address, telephone number, shipping address, requested products and optional notes. This information is emailed to info@lemmo.com so our clinic can call you to confirm the requested items, shipping and payment before finalizing your order. No payment card details are collected through this form.
The patient intake page collects the answers and files you submit, including health information, your contact details, and a typed signature. A completed PDF and any supporting files are packaged in a password-protected archive and emailed to info@lemmo.com for our clinic to review. The email subject includes your last name and date of birth to identify the submission. The website does not add these files to its public media library. You can save a copy of the completed PDF for your own records.
How information is used
Information submitted through these forms is used to respond to your request and coordinate the next step. It is not a substitute for a clinical consultation. Please contact the clinic for instructions on securely providing medical records.
Website features
Essential WordPress cookies may be used for site administration. Embedded videos connect to YouTube or TikTok when you choose to play them; the platform’s privacy terms apply to that interaction. Embedded maps connect to Google Maps and may share standard browser information with Google. This website does not load advertising trackers.
